[MlMt] Remove and restore Fastmail IMAP account in MM

I've been using MM to access my Fastmail account. About a week and a 
half ago, I began getting this dialog, but I couldn't get the mailbox 
online:

After a bit of back and forth with Fastmail support and I'd generated a 
new app password a couple of times, Fastmail support suggested that I 
remove my Fastmail account from MM and then add it back.

I see that one of the right-click options on the account is "Remove," 
which seems straightforward. For when I add the account back, I have a 
couple of questions:

In a spirit of "better stupid questions than stupid mistakes," does the 
dialog above get the 16-character app p/w, or the p/w I use when logging 
into the Fastmail web interface?

I use server-side rules (and now I'm glad I do) to sort mail into 
various mailboxes, so those rules won't go away. Will the smart 
mailboxes, of which I have quite a few, go away? I hope not.
