I have no Inbox rules at all. Here are the Sieve relevant lines in the raw message:

X-Sieve: CMU Sieve 3.0
X-Spam-known-sender: no
X-Spam-sender-reputation: 500 (none)
X-Spam-score: 7.2
X-Spam-hits: HEADER_FROM_DIFFERENT_DOMAINS 0.248, HTML_FONT_LOW_CONTRAST 0.001,
  HTML_MESSAGE 0.001, ME_HAS_VSSU 0.001, ME_SENDERREP_NEUTRAL 0.001,
  RCVD_IN_DNSWL_NONE -0.0001, SPF_HELO_NONE 0.001, SPF_PASS -0.001,
  URIBL_DBL_ABUSE_SPAM 7, LANGUAGES en, BAYES_USED none, SA_VERSION 3.4.2
X-Spam-source: IP='192.40.173.99', Host='mailer99.gate173.rs.smtp.com', Country='US',
  FromHeader='com', MailFrom='com'
X-Spam-charsets: plain='utf-8', html='utf-8'

I also have image blocking check for images in Junk Folder, if that's relevant. I can't see how it is. See image;
