[MlMt] Markdown tables and the Markdown engine in general

I believe that MultiMarkdown and PHPMarkdown Extra are considered pretty
much the definitive sources for this syntax.

However I think that two dashes are used to ‘em-dashes’ which may be why
tables require at least three.
