Runbox can do this.

I use it all the time. The only problem is that Mailmate only lets me define a few addresses. This is a problem when I'm composing a new email. I have to remember to change my From address from ron at runbox.com to ron at mydomain.com. When I'm replying to an email, Mailmate will automatically use my email address that the original email was sent to.
