We should focus on the “throttled” state first. MailMate won't work well as long as this happens since only the Inbox is really synchronized. Throttling most often happens for large accounts. I recommend that you change the synchronization schedule of most of the mailboxes in the account to “Manually” (Mailbox ▸ Synchronization Schedule). The exception should be mailboxes you often use in other email clients and/or devices. These mailboxes will still be synchronized when you select them or, e.g., move messages into them.

You can expand all mailboxes in the account by holding down ⌥ when you click the triangle (if you have a large hierarchy of mailboxes). You can use the shift key (⇧) to select a large range of mailboxes.

At first, you might even want to take most of your mailboxes offline using “Mailbox ▸ Take Offline”. When the account is no longer throttled then you can take more and more mailboxes online.

If this doesn't work then it might be because there is some kind of busy-loop issue with a specific mailbox. In that case, see if you can isolate the problematic mailbox (by taking everything else offline). Then we can generate logs to learn more about this mailbox.

When this works we can move on to your other points.

Here we'll focus on the Outlook account. I'm assuming the emails show up in a specific mailbox? My guess is that MailMate is trying to upload a message. This “succeeds” without MailMate being aware of it and therefore it retries repeatedly. This can happen if the server breaks the connection for some reason immediately after each upload. That's just a guess.

See if you can reduce the problem to a specific mailbox and/or message. It might help, again, to take most of the mailboxes offline. You can keep the Activity Viewer (⌥⌘0) open. It might not make much sense to you, but you can see what is currently active.
