I think they differentiate between messages sent to (or from) just the 
user, or the user and others, or a mailing list. That's unnecessary and 
way too cluttered for me, though. ShowInOut makes the images 
user-controllable (if you're comfortable editing the userChrome file, 
which I am); I just put a png image of "to: " before messages I send, 
which makes it very easy for me to scan the columns and know what my 
correspondence looks like. Here's a pic:

(My eye does prefer having no mark on the messages I send; it makes the 
difference between from-me and to-me stand out.)
